The answer is simple: According to the Koch mindset, and Charles Koch owns most of the GOP in federal and state offices, if you can't pay for ALL of your health care out of pocket, you deserve to die. Koch is a sociopath who thinks he is a visionary like Martin Luther. He thinks that anybody who works for anybody else, let alone the poor and the aged, is a "parasite." Classic projection, which is what sociopaths do. He thinks he is intellectually superior to everybody not in his income level, that he made "his" money all by his little self and without any input from his daddy Fred Sr., his employees, the people who consume his products, and the politicians who give him taxpayer handouts. If these politicians could get away with it, they will get rid of Medicare, Medicaid, and employer-paid insurance.

Edited to add this little tidbit: Charles Koch should read the DSM-V manual. Here is a list gleaned from Psychology Today describing the traits of somebody with narcissistic personality disorder (NPD), which Koch clearly has:

1. Has a grandiose sense of self-importance.
2. Is preoccupied with fantasies of unlimited success, power, brilliance, beauty, or ideal love.
3. Believes that he or she is "special" and unique and can only be understood by, or should associate with, other special or high-status people (or institutions).
4. Requires excessive admiration [regularly fishes for compliments, and is highly susceptible to flattery].
5. Has a sense of entitlement.
6. Is interpersonally exploitative.
7. Lacks empathy: is unwilling [or, I would add, unable] to recognize or identify with the feelings and needs of others.
8. Is often envious of others or believes that others are envious of him or her.
9. Shows arrogant, haughty [rude and abusive] behaviors or attitudes.

Virtually all of these describe Koch. Dangerous, pathologically crazy individual.
